Reviews and consultation

Records and prior-opinion reviews

A focused review of selected records and one clear clinical question—without starting with a full evaluation.

01 / WHEN IT MAY FIT

What this service can help answer.

A VA mental health records review focuses on information already available, such as treatment records, a prior C&P examination, DBQ, private opinion, or relevant decision documents. It can help define the clinical question, identify important gaps or conflicts, and assess whether a fuller evaluation may add useful information. It is not automatically a diagnosis, nexus opinion, or legal case review. The records organizer can help you prepare a focused file set.

  • A prior examination contains possible errors or missing information.
  • Several opinions reached different conclusions.
  • A veteran wants to understand what the current evidence supports.
  • It is not clear whether a full evaluation would add useful evidence.

02 / WHAT IT INCLUDES

A clear scope before work begins.

The plan names the exact records, one clinical question, and whether the agreed deliverable is a written summary, consultation, or explanation of possible next steps. Added files or a broader question may require a revised scope before review continues. See how records are prepared and reviewed for the larger process.

03 / SCOPE

What this service covers.

This service can organize the clinical evidence and timeline, examine the methods and reasoning in prior opinions, and identify what remains unanswered. It does not include an interview, new diagnosis, nexus opinion, or DBQ unless those services are expressly added. When a new clinical assessment is needed, an independent psychological evaluation is the appropriate service to explore.

Included in the scope

  • Organize the relevant evidence and timeline
  • Identify important gaps, conflicts, or unanswered questions
  • Explain whether a fuller evaluation may be useful

Outside this scope

  • Include an interview, full evaluation, nexus opinion, or DBQ unless added to the plan
  • Choose an appeal or decision-review path
  • Reach a new diagnosis without a clinical evaluation
